Liquor Licenses

No person, firm or corporation shall vend, sell, deal, or traffic in or have in his possession with intent to vend, sell, deal or traffic in or, for the purpose of evading any law or ordinance, give away any intoxicating liquor or fermented malt beverage in any quantity whatever, or cause the same to be done, without having procured a license, nor without complying with all provisions of Section 2, Chapter 7, City Ordinances, and all statutes and regulations applicable thereto, as from time-to-time amended.

Print or type in ink. Provide a complete answer for each question. Applications must include your Sellers Permit Number and FEIN.

NOTE: If there are any outstanding invoices due or compliance matters with the City, North Shore Health or Fire Departments, you may not be approved until those invoices are paid in full and compliance is obtained.

Additional Requirements 

If you have a change of agent only, you will need to complete the AT-200 Appointment of Successor Agent Form and submit this to the City Clerk's Office in accordance with Wis. Stats. 125.04(6).

Fees

$100.00 Class "A" Fermented Malt Beverages - Retail Sales/Consumption Off Premises
$100.00 Class "B" Fermented Malt Beverages - Retail Sales/Consumption On Premises
$100.00 "Class C" Wine - Retail Consumption On Premises
$500.00 "Class A" Intoxicating Liquor - Retail Sales/Consumption Off Premises
$500.00 "Class B" Intoxicating Liquor - Retail Sales/Consumption On Premises
$10.00 Change of Agent 

Additional Fees

$25.00 Publication Fee for every applicant
$15.00 background investigation report for each person named on application.

Late Penalty

A double fee will be assessed on all delinquent applications.

Duration

License shall have a term of one (1) year commencing on July 1st and expiring on June 30th of each year. If granted subsequent to July 1 in any given year, license shall expire on June 30 after issuance. Renewal applications are due by May 15 in the year of expiration.
