Food Licenses

Title 8, Chapter 4, Glendale Code of Ordinances, provides that no person, firm or corporation shall keep, store, process, manufacture, offer for sale, sell or dispense food, milk, or milk products, in any building or upon any premises in the City of Glendale unless first licensed to do so for each location at which food, milk, or milk product is so kept, stored, processed, manufactured, offered for sale, sold or dispensed.

Limited Food Service - $100.00: A retail establishment serving only individually wrapped, hermetically sealed, single servings supplied by a licensed processor.

Simple Complexity Food Service - $175.00: A retail establishment where ALL of the following criteria are met:

• Food is prepared to order.
  
• Food may be kept in a hot or warm condition, but only for one meal period. Such food may not be cooled and re-served at a later time.
  
• There is only limited preparation of food, such as slicing onions or pickles as condiments, or mixing pre-packaged products.
  
• Potential for cross-contamination must be minimal. No raw chicken or other meats may be used. Pre-breaded fish or preformed burger patties (fresh or frozen) may be used.

Moderate Complexity Food Service - $250.00: A retail establishment where ANY of the following criteria are met:

• Any potentially hazardous food (as defined in Wis. Admin. Code) is cooled and/or reheated.
  
• Any raw poultry, seafood, or bulk beef is served or sold. (This does not apply to use of preformed patties.)
  
• Any food products are made from scratch.
  
• The facility has walk-up service or a drive-up window.
  
• The facility seats 50 or more patrons.
  
• The facility provides delivery of food.
  
• The facility provides catering services.
  
• The facility provides banquet services.

High Complexity Food Service - $350.00: A retail establishment in which five or more of the moderate complexity criteria, as set forth above, are present.

All Other Food Service - $350.00: An establishment that requires a license for food production, distribution, processing, storage, etc. that does not meet any of the aforementioned definitions.

Additional Fees: Greater than 2 Inspections, add $100 per Inspection

Late Penalty: A double fee will be assessed on all delinquent applications.

Duration: License shall have a term of one (1) year commencing on July 1st and expiring on June 30th of each year. If granted subsequent to July 1 in any given year, license shall expire on June 30 after issuance.
