Change of Mailing Address

  1. We can only process adress change requests submitted by the property owner. If you are an authorized agent of the owner, or have Power of Attorney, you must attach the appropriate legal document to the request or it will not be processed.
  2. This form is used to change a MAILING ADDRESS ONLY. Names are not changed unless a conveyance document has been recorded with the Milwaukee County Register of Deeds Office. (We do process these changes automatically on a monthly basis.)
  3. This address change will remain in effect until we receive a subsequent change request form, or after title is transferred.
  4. The mailing address you provide will be used for ALL City of Glendale correspondence, tax bills and assessments notices.

    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, 3) you may still be required to provide a traditional signature at a later date, and 4) you understand that failure to receive mailings from the City of Glendale has no effect on their validity or your responsibilities as a property owner.
